ABOUT Dr. Derickson: Dr. Derickson was born in Tucson, Arizona. He attended Tucson High School, the University of Arizona and received his Dentistry Degree from the University of Southern California where he graduated in the top 2% of his class. Dr. Derickson has received numerous honors including Who's Who in Arizona, Who's Who in the West, Who's Who in America, Who's Who in Executives and Professionals, America's Top Dentists, and Top Dentists in Tucson (as voted by his peers). He is also a member of the Fiji Hall of Fame! He belongs to the Catalina Rotary Club and is a Past President. His hobbies include hunting, fishing, skiing, knife making, photography and tennis. His father was an orthopedic surgeon in Tucson. He has 2 brothers and 2 sisters. Dr. Derickson is married and has three daughters and 6 grandchildren. A life-long member of the Southern Arizona Dental Society and the American Dental Association, he is constantly attending continuing education classes to achieve an even higher level of knowledge and expertise. He is a Laureate of the American Academy of Dental Sciences and Fellow of the American College of Dental Sciences.

ABOUT Dr. Dalesandro: I love Tucson. I was born and raised here and have come to really enjoy the people and surroundings. I was raised by Sam and Ronnie Dalesandro who both live here in Tucson. I grew up swimming and playing soccer and was quite an athlete.

I attended Salpointe Catholic High School and graduated in 1997. As you know, I met my future wife Debbie there in Homeroom. We swam on the varsity swim team together and attended prom together our senior year.

It was while at Salpointe that I found my love for Dentistry. During my senior year I took a course that allowed me to observe a professional in our community. I decided to work with Dr. Ronald Quintia, a Maxillofacial and Oral Surgeon. During that time I was amazed at what one could do for others and fell in love with the ability to make people happy through the power of healing. Dr. Q. advised me to take business courses because they wouldn't teach you that in dental school.

Off I went to the University of Southern California. I graduated in 2001 with a degree in Real Estate Finance and Development. The Marshall School of Business at USC is one of the top business schools in the country and I am very proud to have earned my degree from there.

I moved back to Tucson after graduation and worked for a national home-builder. I started in site sales and acquired many tools about communication and interaction with others. As my professional career was blossoming Debbie had moved back to Tucson and our relationship together evolved to talks of marriage and family. Watching her father, Dr. Derickson, treating patients in his office with such skill and enthusiasm rekindled my desire for Dentistry as a career. It was then that I decided to reinvest myself in school and pursue my doctorate degree in Dentistry.

Debbie and I moved to Glendale, Arizona, where I earned my Doctor of Medical Dentistry degree. Along the way we had two beautiful children, Lincoln and Kason. Although they were born in Phoenix, they will be raised as Wildcat fanatics just like Mommy and Daddy.

HOURS OF OPERATION:

Monday – Friday 8:00 AM to 5:00 PM

Emergencies: Call the office for the emergency number or the dentist that is on-call.

New Patients: Please arrive 15 minutes early for paperwork. This will aid us in seeing you on time for your appointment.

BRING TO YOUR DENTAL APPOINTMENT:

- An up-to-date list of your medications( including vitamin supplements). - An up-to-date list of your medical conditions and allergies. - Information about your health care providers, including all of your doctors and their phone numbers (including previous dentist). - Information about your emergency contacts. - Your dental insurance or medical cards. - Your dentures or partials...even if you don't wear them. OR - Download' New Patient Forms' and bring them with you completed.

Important: Insured patients must have all necessary information so that we can request benefit information from your insurance carrier. This includes your insurance card and/or the requested insurance information on the insurance section of the New Patient Form.

OFFICE POLICIES:

FEES/APPOINTMENTS: Our desire is to make our fees reasonable and affordable for all of our patients. Your visit here is not about money but is about your health. Every effort is made to seat and dismiss our patients on time. We reserve time for each patient in advance unless a patient is in pain and has an emergency need. We would appreciate your cooperation in making and keeping your appointments.

FINANCIAL POLICY: Payment is expected at time of service. We inform all our patients about the necessary treatment and the fees for each prior to any treatment. We will accept Master Card, Visa and Discover as payment for your dental charges. We do not ever want finances to stand in the way of your health. We are able to introduce you to several options for small personal loans to assist you. Upon approval, a limited line of credit may be arranged within our office. Please ask us how we might help you.

INSURANCE: As a courtesy to your patients we are happy to submit claims for payment to your insurance company. Be advised that although we strive to have accurate benefit information, changes in benefits can occur at any time. Calculated payment portions are only an estimate based on benefit information available on a day of calculation. The patient or guardian is ultimately responsible for any uncovered treatment costs.

CANCELLATIONS: We endeavor to schedule appointments as to allow sufficient time for the doctor and/or hygienist to provide quality care. This limits how many appointments we accept in one day. We ask for 48 WORKING hours notice if you need to cancel or reschedule an appointment. Doing this will help you avoid being charged a cancellation fee of $38. Enforcement of this policy is necessary in maintaining a high quality of patient care. We appreciate your support in this regard.

COPY OF RECORDS: We are always sad to hear that you are moving. Unlike many offices, we provide our patients with a copy of their records free of charge. We only request that you fill out a records release form. This protects your privacy and ensures that they go to the correct party.

STERILIZATION/INFECTION CONTROL: We can never do enough to protect ourselves and our patients from potential hazards. With all the recent concern regarding the risk of infectious diseases in the dental office, we want you to know that we are going to make your dental environment as safe as possible.

  1. We are heat sterilizing hand pieces
  2. We use disposable items (all injection materials, cotton, gauze, polishing cups for cleaning teeth, to name some) which are discarded after one use.
  3. We use gloves, masks, and protective eye wear.
  4. We use disinfectant sprays, solutions, and hand soap for sinks, counter tops, chairs, etc.
  5. We wear protective barrier uniforms, which are laundered and disinfected daily.
  6. We meet or surpass all of the O.S.H.A. (Occupational Health and Safety Administration) guidelines, and continue to learn and upgrade our cleanliness and sterilization techniques.

PREMEDICATION: If you have a heart murmur, prosthesis (hip replacement), or implant of any kind, please consult your physician regarding any premedication and appropriate antibiotic that may be necessary prior to your dental visit. This is important information that we need to know about prior to your treatment in our office.

INITIAL EXAM: Our new patient exam is thorough and complete. We are responsible to you about all your dental health needs as well as what is available cosmetically. Our exam consists of looking for:

  1. Recurrent decay (decay under old fillings)
  2. New decay
  3. Gum and bone disease
  4. Oral cancer
  5. Tumors
  6. Cysts/lesions
  7. Abscesses
  8. TMJ (jaw joint disorders)
  9. Sensitivity
  10. Orthodontic issues
  11. Fractures
  12. Bite problems
  13. Cosmetic needs
  14. Reviewing your medical history

RADIOGRAPHS: We will recommend a complete set of x-rays on all new patients to provide you with a thorough and comprehensive exam. These x-rays will show us the following:

  1. Decay
  2. Fractures
  3. Tumors/cysts/lesions/abscesses
  4. Irregularities
  5. Root Tips
  6. Bone loss
  7. Overhangs (fillings that are ill fitting)
  8. Hard deposits under your gums
  9. Impacted teeth

If you have x-rays from another office, please let us know so we may obtain and use them. This enables us to take only minimal amount of new films.

RECARE APPOINTMENTS: We strongly believe that the greatest degree of oral health care can be obtained only through adequate patient home care with regular, periodic, professional cleaning and recare visits. This way, many problems can be prevented and corrected before they become extensive. Our office has developed a recare system customized for each individual patient's cleaning needs. We look at the symptoms you are experiencing to determine the "type of cleaning" you will need. There are many factors contributing to your symptoms other than just your oral hygiene. Pregnancy, stress, medications, diet, ill fitting crowns and bridges, tobacco, and occlusion are a few of these factors. Your treatment needs can change quickly. We know that a healthy mouth requiring a "regular cleaning" is void of bleeding, swelling, odor, calculus (tartar) and bone loss. Your individual needs will always be discussed.
